The HSE in Dublin North City and County is looking to hear from parents about school vaccinations. 

This survey is about the 4-in-1 vaccine which protects against diphtheria, polio, tetanus and whooping cough  and the MMR vaccine which protects against measles, mumps and rubella.

Please help us understand what we are doing well and where we can improve.
