Thank you for your interest in participating in the Tobacco Free Campus Quality Improvement Bursary for 2026/27
The purpose of this survey form is to capture your initial audit score and descriptions of the actions you will implement as part of your quality improvement plan (QIP) to improve tobacco management within your service.
This self- audit and QIP builder is based on the Global Network for Tobacco Free Healthcare Services Best Practice Standards.
Most of the questions require Yes / No responses.
You are then asked to complete the free text box (total of 8) where you will outline actions that you intend to implement to improve the standard in question.
Please include enough detail so that we can understand the action(s) to be taken/change(s) to be made.
We recommend that the identified actions and scores allocated for each standard be discussed by your Tobacco Free Campus or Healthy Ireland Committee and agreed as a group before being completed on this survey form. You can access a printable version of the survey and QIP template here that you can use to guide discussion.
